Therapeutic approach to metastatic myelo-lymphoproliferative malignancies in spine

Abstract

Introduction Myeloproliferative metastatic tumors (leukemia, multiple myeloma and lymphoma) are a problem that the spinal surgeon faces increasingly due to longer survival periods associated with their slower growth rates and improved oncological treatments available.Currently there is not consensus about any classifications that establish a prognosis and a treatment plan for the management of these pathologiesObjectiveTo apply and evaluate specific survival scale (ECOG and SCMMM) and treatment (Sins) for this type of spinal metastases. Material and MethodsPatients with diagnosis of metastatic spine myeloproliferative diseases had the above scale applied and also adding visual analog scale (VAS) and Oswestry (ODI) to quantify the evolution of treatment. Were included in this sample 5 patients with a minimum follow up of 6 months. ResultsOf the patients sampled 4 patients had a diagnosis of multiple myeloma and one a diagnosis of leukemia, 2 were operated and 3 were treated with radiotherapy, chemotherapy and corset; all improved EVA and Oswestry except one who died at 7 days by an extraspinal complication. ConclusionThe scales used are very useful for therapeutic decision of these patients to be more specific for these metastases.
